The Evernorth® Behavioral Health provider network
Thank you for your interest in joining the Evernorth® Behavioral Health provider network. Your participation with us helps fulfill our mission to improve the health, well-being, and sense of security for the people we serve. Here you will find instructions on how to join the Evernorth network as an individual provider, clinic, and/or facility.
Evernorth network providers must treat all covered patients equally and must agree to participate at all service locations. Participation is not limited to specific Tax Identification Numbers (TINs).
How to join
Individual providers
Evernorth defines individual providers as independently licensed behavioral health practitioners who contract directly with Evernorth. This category includes providers opening a new practice, joining an existing practice, or affiliating with a contracted clinic or facility.
The following are required per provider. Note that all items must be current, active, and in good standing.
A license to practice independently (without supervision) in the state in which you are providing services
For prescribing providers only: A current Drug Enforcement Administration (DEA) license for all locations in which you will be providing services (expiration date must be at least 60 days away)
Proof of Professional Liability and General Liability insurance with the following minimum coverage limits:
- $1 million/$3 million for physicians and psychiatric nurses with prescriptive authority, or
- $1 million/$1 million for all other providers
Exceptions include:
- Texas: The minimum recommended amount of professional liability coverage for providers in Texas is $200,000 per occurrence and $500,000 aggregate.
- Florida: The minimum recommended amount of professional liability coverage for providers in Florida is $100,000 per occurrence and $300,000 aggregate.
- Providers in states which regulate a patient compensation fund for insurance coverage, or which regulate the amount of insurance a provider must carry, should meet at least the state's requirement if they do not meet Evernorth Behavioral Health minimum requirements.
- Providers with Federal Tort Claims Act (FTCA) coverage are not required to meet the Evernorth Behavioral Health minimum professional liability insurance requirements. Providers claiming this exemption must furnish either a copy of their FTCA coverage letter or an attestation confirming that they are covered under the FTCA.
A Council for Affordable Quality Healthcare (CAQH®) identification number
Evernorth relies on CAQH to verify provider information throughout the contracting process. To avoid delays, ensure the following information in your CAQH profile is complete, current, and accurate:
- Current attestation: Must re-attest every 120 days, or every 180 days for Illinois providers.
- Authorization: Evernorth must be authorized to access your CAQH profile.
- Current W-9 matching the tax ID submitted with your Provider Information Form
- Professional license: Must be active in the state(s) where services are provided.
- DEA Registration (if applicable): Must be active in the state(s) where services are provided.
- Education: Graduate education information must be listed, including school name, degree, and start/end dates. Internship, residency, or fellowship information should be included when applicable.
- Primary specialty: Your primary specialty must be listed.
- Professional liability insurance: Coverage must be current and meet applicable minimum requirements.
- Work history: Five years of consecutive work history must be listed with explanations for gaps over six months.
- Current Resume or CV (if used): Must be dated within the last two years.

Evernorth offers individual providers a streamlined contracting and credentialing process through the Evernorth Provider Portal (see below for exceptions). By completing the Evernorth Behavioral Health Provider Information Form in the portal, you can track your application status and receive notifications if additional information is needed.
Important: Once you begin the application, it must be completed in a single session. You will not be able to save your progress or return later to finish.
After logging in, users with the contracting entitlement will be able to apply by selecting “Contracting” under the My Practice header.
The process to join the Evernorth network can take up to 90 calendar days to complete (or as otherwise required by law). If you work with a third-party agency or clearinghouse to assist with contracting or credentialing, please ensure that they’re aware of this application timeline.
In certain circumstances, providers may receive provisional credentialing, allowing them to begin seeing patients before the full credentialing process is complete. Eligibility is determined on a case-by-case basis and must meet applicable regulatory and Evernorth requirements.

These exceptions apply to individual providers rendering behavioral health services at a participating Cigna Healthcare® medical practice:
If you are providing behavioral services at a participating Cigna Healthcare medical practice and are not accepting direct referrals from the community, call 1 (800) 882-4462 to join the network under the practice’s medical contract.
If you are providing behavioral health services at a participating Cigna Healthcare medical practice and are accepting direct referrals from the community, and/or you provide behavioral health services outside of the medical practice (e.g. in your own private practice), apply to join the Evernorth Behavioral Health network by submitting the Evernorth Behavioral Health Provider Information Form through the Evernorth Provider Portal.
Important notice for Texas only:
Cigna will accept applications for participation in their networks of physicians and other providers from February 1, 2026 through February 28, 2026. Applicants must meet all credentialing and other participatory criteria. All applications will be reviewed and responses will be made within 90 days of receipt.
If you practice in Minnesota, North Dakota, or western Wisconsin:
In these markets, Evernorth has an alliance with HealthPartners for managed care services and Evernorth contracts for Employee Assistance Program (EAP) services only.
- Health Partners: For contract consideration, complete the HealthPartners Behavioral Health Contract Application available at the HealthPartners Provider Portal
- Evernorth Employee Assistance Program (EAP): For contract consideration, apply through the Evernorth Provider Portal
If you practice in Maryland, Ohio or Washington:
As of September 1, 2026, providers in these states must apply through the Evernorth provider portal. See the "How to Apply" section above.
Individual providers

Clinics
Evernorth defines clinics as groups of four or more independently licensed behavioral health providers who share the same tax identification number (TIN), location, and administrative staff. If your practice does not meet this description, please apply to join the Evernorth network as an individual provider.
The following are required per service location and clinician. All items must be current, active, and in good standing. No provisional, conditional, or temporary approvals will be accepted.
Applicable service location form(s):
- Physical Service Location Form
- Home-Visit-Only Service Location Form
- Telehealth-Only Service Location Form
Professional Liability and General Liability insurance policy fact sheet or certificate of insurance that identifies the clinic named in the application and indicates the liability limits and expiration date (may not be binder policies). Evernorth requires the following minimum coverage limits:
- Physicians and psychiatric nurses with prescriptive authority: Minimum coverage of $1 million per occurrence and $3 million aggregate
- All other providers: Minimum coverage of $1 million per occurrence and $1 million aggregate
- State-specific requirements:
- Texas: Minimum recommended coverage of $200,000 per occurrence and $500,000 aggregate
- Florida: Minimum recommended coverage of $100,000 per occurrence and $300,000 aggregate
Additional guidelines: Providers in states with regulated patient compensation funds or specific insurance coverage requirements must meet at least the state's minimum requirements if they do not meet Evernorth Behavioral Health standards.
Providers with federal tort coverage (Federal Tort Coverage Act) are exempt from Evernorth Behavioral Health minimum insurance requirements. These providers must include a copy of the federal tort letter or an attestation of coverage in their credentialing.
A completed W-9 for each TIN
- Complete the Screening Application for Behavioral Health Clinics
- Email the application and required documents to the Evernorth Behavioral Health Clinic Contracting Unit at BehavioralOutpatientClinic@Evernorth.com
Once a complete application is received, Evernorth will review the materials and contact the clinic regarding next steps or to request any additional information. Our communications will be sent to the email address listed for the "Primary Clinic Contracting Contact” in the application.
Clinics
If you have any questions during the clinic application process, please email your recruiter or BehavioralOutpatientClinic@Evernorth.com and include your National Provider Identifier (NPI) and TIN on all correspondence.
Facilities
Evernorth defines facilities as organizations delivering higher levels of care, such as facility-based inpatient, residential, and partial hospitalization programs (PHP); intensive outpatient programs (IOP); detoxification services; and other licensed behavioral health programs that bill on a UB-04 claim form.
A UB-04 claim form supports reimbursement through bundled, per diem rate methodologies. Organizations that bill exclusively on a CMS-1500 claim form may apply to join the Evernorth network as a clinic or individual provider.
The following are required per service location. All items must be current, active, and in good standing. No provisional, conditional, or temporary approvals will be accepted.
A valid state license, certificate, or permit to operate
Proof of professional liability and general liability insurance with the following minimum coverage limits:
- Residential/inpatient facilities: $1 million per occurrence/$3 million aggregate
- If PHP/IOP only: $1 million per occurrence/$1 million aggregate
Behavioral health accreditation (e.g., Joint Commission, CARF) and/or Medicare certification
A completed W-9 for each tax identification number (TIN)
- Complete the Evernorth Behavioral Health Facility Information Form
- Email the form and required documents to BehavioralFacilityRecruitment@Evernorth.com
Once a complete submission is received, Evernorth will review the materials and contact the organization regarding next steps or if any additional information is needed.
Note: Submission of a Evernorth Behavioral Health Facility Information Form and supporting documentation does not guarantee contracting consideration or network participation. Evernorth evaluates requests based on network needs, regulatory requirements, credentialing standards, and business considerations.
