Provider profile and directory changes

If you’re making changes for or as an individual provider, you have two update options, depending on the type of information that needs to be changed.

Use Provider.Evernorth.com to review or update service addresses, National Provider Identifier (NPI) numbers, office email addresses, appointment scheduling phone numbers, licenses, and whether you’re accepting new patients:
1. Log in to Provider.Evernorth.com. 
2. Select My Practice. 
3. Select Directory Update. 
4. Review, update, and confirm your information.

Your portal role determines what information you can update. If you don’t see the Directory Update option, contact your organization’s website access manager for help updating your access.
If you haven’t registered for the portal yet, you can register for secure 24/7 access. After registering, you can check eligibility and benefits, view remittance reports, enroll in Electronic Funds Transfer (EFT), and update your directory profile.

Call Provider Services for help with individual provider names, clinic or facility affiliations, websites, Tax Identification Numbers (TINs), all other change types, or for adding a new provider or practice location to a currently contracted group:
1. Call Provider Services at 1 (800) 926-2273. 
2. When prompted, say “health care professional” and then select prompt number 4 for demographic updates.

To add, change, or remove service location information, submit the Clinic Location Changes for Existing Clinic Contracts Form by email or fax.

For help adding or changing service location information, or adding or removing services:
1. Call Provider Services at 1 (800) 926-2273. 
2. When prompted, say “health care professional” and then select prompt number 4 for demographic updates.
