Evernorth Care Group’s highly specialized and integrated health care team work together to connect care for your customers living with chronic conditions such as diabetes, hypertension (high blood pressure), hyperlipidemia (high blood pressure), and chronic obstructive pulmonary disease (COPD). These teams include the following:
Care Management: Team members include nurses (RNs and LPNs), social workers and non-clinical care navigators specially trained to help manage and connect care for high- and rising-risk customers. They focus on care coordination, chronic disease management and social determinants of health.
Care Navigators: Connects customers with programs and services; and collaborates with clinicians and other members of the team to improve customer care.
Clinical Pharmacy: These pharmacists (PharmDs) focus on chronic disease and medication management, medication adherence and reconciliation. They operate special disease-specific programs, including diabetes, anticoagulation-related conditions, and HIV-PrEP.
Behavioral Health Managers (BHMs): Support holistic whole-person health with a focus on healthy behaviors to connect care. BHMs offer your customers evidence-based treatment options for brief interventions focused on overcoming adverse circumstances and being fully engaging in the management of their health. BHMs customize treatment options according to customer and primary care goals. Techniques such as mindfulness, cognitive therapy and motivational interviewing are often used. BHMs also collaborate with specialty behavioral community partners including Crisis Preparation and Recovery (CPR), to assist customers in ongoing behavioral care.
